iQ HealthMaps provides a comprehensive understanding of the social and ethnic differences in practice populations and their health needs, unmasking hidden detail to deliver cost-effective interventions in patient care, business modelling and better commissioning.
This is achieved by extracting patient data from all GP clinical systems, secondary care data sources such as SUS, socio-demographic information from Experian’s Mosaic Public Sector and combining this in an intuitive, interactive mapping tool which uses web protocols to empower all parties involved in public health.
With the emphasis now on prevention of ill health, GPs are at the frontline of providing services that keep people healthier longer as well as reducing demand for secondary care (and thus helping to alleviate significant overspends in PCT allocations). With the full functionality of HealthMaps predictive risk modelling can be implemented.
GP
clinical records are the most valuable repository of information about
population health
iQ HealthMaps integrates directly with all
clinical systems to extract patients’ postcodes and Read Codes for health
status mapping. Even data which may have not been coded correctly in the
clinical systems is extracted using ‘fuzzy logic’ search to ensure that no
patient records are missed. Only those responsible for the primary care of
patients can access patient identifiable data.
Socio-
demographic data
Experian’s Mosaic Public Sector classifies
all people in the
into 61 types and 11 groups to give a clear insight into their
socio-demographics, lifestyles, deprivation, education, culture and behaviour.
This provides the most comprehensive view of patients and the issues affecting
their health.
Locations and details of GP practices, hospitals, health centres, pharmacies, dentists and clinics are all included to provide a clear picture of health resources.
Geographic
boundaries
PCT, Localities, PBC clusters, GP boundaries, Wards, Postcode Areas and derivation levels are all incorporated. ‘True’ GP boundaries can be created by plotting the postcodes of the practice patient list.
